Description
Responsibilities:
- Deliver specialist nursing care in defined area of nursing in accordance with national and local policy and guidance.
- Manage and co-ordinate a complex nursing caseload within Livewell SW.
- Act as a Care Manager/Care Co-ordinator for patients/carers, commissioning care packages and planning to promote positive risk management.
- Assess, diagnose, implement, plan, and review complex packages of care in partnership with patients, carers, and multi-professional agencies including case conferences.
- Provide specialist advice utilizing health promotion and behavior change skills to optimize the health of patients with life-limiting illnesses.
Clinical Support:
- Provide expert advice and clinical support while promoting effective delivery of high-quality service within the community.
- Offer professional clinical support to the nursing team; advise on practice.
- Identify training needs of staff working in partnership with clinical operational managers; encourage practitioners to develop critical thinking skills.
Advanced Nursing Practice:
- Use Health Needs Assessments and change management tools for health promotion initiatives.
- Work within NMC professional code of practice; be flexible across the organization as needed (e.g., during a pandemic).
Prescribing Responsibilities:
- May serve as a non-medical prescriber/Nurse Prescriber following local Joint Formulary guidelines.
Holistic Care Planning:
- Apply advanced skills for holistic assessments; develop care plans collaboratively with individuals having complex conditions using an evidence-based approach.
Qualifications & Skills:
- Hold a Specialist Practitioner Qualification enabling advanced clinical assessments.
- Provide expert advice while supporting MDT meetings involving multi-professionals.
- Supervise standards of care delivered by the multidisciplinary team.
Operational Management Duties (if applicable):
- May act as operational manager or deputize in their absence.
- Manage coordination of complex caseloads alongside operational manager; support delegation tasks among team members.
Communication & Collaboration:
- Maintain strong relationships with GPs, social services, mental health professionals, acute hospitals etc.
- Communicate effectively regarding changes to prescribing interventions or treatments provided to service-users.
People Management Tasks:
- Undertake recruitment processes including induction training; manage performance-related tasks such as appraisals or grievances.
- Plan supervision/teaching experiences for nurses' learners according to organizational policies.
Resource Governance:
- Demonstrate awareness regarding cost-effective usage/resources while ensuring compliance with funding eligibility criteria (e.g., Care Act).
- Authorize supplies/expenses related duties including off-duty arrangements or procurement needs for the team.
Incident Reporting:
- Report all incidents related to health/safety/security concerns promptly using appropriate channels ensuring timely documentation is maintained through IT systems.
