Description

Main Responsibilities:
- Take responsibility for the project management, development, and delivery of a portfolio of projects and work-streams across a range of areas, which focus on quality, resilience, improvement, and transformation.
- Lead on transformation and innovation of services across the practice and PCN.
- To lead on strategic practice developments and manage the implementation of significant projects for the practice including relationships with other practices and agencies regarding improved healthcare provision.
- To be responsible for ensuring that the practice delivers appropriate healthcare through understanding local population needs and to work with the partners and other key players to ensure that services change to meet the needs of the local population.
- Lead teams and support clinicians and managers to review current service models / delivery with a view to improving and transforming through best use of resources to provide better, faster, and safer care.
- Support the senior management team with writing business development plans.
- Attend cross organisation meetings as required.
- Write and submit project reports for the Board as required, showing progress against milestones, status, issues, risks.
- Oversee rollout/delivery of new services alongside Head of Operations/Operations Managers.
- Be a champion for best practices; learn from experience/others; support spread of innovation within Practice.
- Develop programme/project management tools/reports/templates transferable between projects usable throughout project lifetime from design to delivery.
- Assist Senior Leadership Team with capacity planning review ensuring appropriate healthcare delivery through understanding local population needs working with partners/key players ensuring service changes meet those needs.
- Maximise income generating potential through feasibility/scrutiny new service opportunities working with team members ensuring efficient/effective running meeting service delivery targets.

Transformation:
- Develop plans for projects identified by Senior Management/Operational Teams utilising support from PCN Business Analyst/Operations Managers supporting implementation projects.
- Work with PCN Business Analyst/Operations Manager ensuring Practice has full suite data interpretable/utilised improvement driving strategy supporting population health management.
- With direction from Senior Management engage ICB Primary Care/Digital teams ensure local transformation plans/activity aligned ICB priorities e.g., improving access; PCN Development.
- Provide feedback management team progress current initiatives capacity change issues so that plans can adapt response.
- Support programme audits/surveys evidence measures improvement undertake full evaluation cycle new services/service improvements.

Governance:
- Ensure practice complies NHS contractual obligations relation patient care engage/contribute processes ensure Group meets Care Quality Commission requirements specifically safe staffing levels equipment levels room conditions at all times contribute development maintenance sound clinical governance/risk management within practice lead clinical governance/audit ensure all clinicians conform clinical guidance protocols report audit results have oversight groups clinical audit cycle ensure clinical safety reports are in place hazard risk logs completed prior deployment new services/new technology completion required safety documentation i.e., DSB160 working Head Operations review learning/transformation requirements form reported SIs identifying improvements required maintain accurate documentation relation standard operating procedures contracts contacts decision making identify record risks/issues developing contingency plans service contract leads responsible delivering robust information systems relation project documentation (dashboards highlight reports risks/issues logs).

Research Management:
- Engage/support GP lead research develop external partnerships relation clinical research lead clinical trial setup site feasibility site initiation provide ongoing management support duration each trial ensure sound robust system tracking visits associated income ensuring visits unscheduled activities invoiced develop processes track progress pipeline trial activity feasibility study completion maintain oversight research governance requirements lead identification new studies assessment feasibility oversee process/resource ensure smooth running studies within team ensure trial/grant conditions adhered including provision milestone reports etc., association research staff.

Finance:
- Monitor/report resource spend return investment tracking against planned milestones responsible research funding budgets continuous monitoring viability research modules maintain budget any research projects have oversight/work operational management project budgets ensure no overspend NHS monies manage delegated budgets propriety line relevant organisational procedures take responsibility delivering research services associated income stream appropriate role monitor track budget allocation delivering research PCN enhanced services ensuring service delivered within budget.

Leadership:
- Nurture collaborative productive relationships between teams foster culture continuous improvement delegating decision making far appropriate support represent Group external meetings workshops as appropriate build effective positive relationships colleagues patients other partners.

Operational:
- Support Board Director facilitate key operational delivery event system disruption including initiation business continuity plan.

Communication:
- Support building relationships facilitate collaboration between PCNs practices wider system supporting delivery care patients maintaining credibility key contacts/system colleagues translate/present information clear formats written documents presentations ensuring understanding range audiences be responsible sharing information/best practices persuading/influencing change services clinicians range stakeholders develop/maintain effective communications across Practice relevant outside agencies efficient effective running Practice site post holder need use persuasive/motivational skills achieve improvements/services resistance change foster collaboration across teams/stakeholders build effective positive relationships colleagues patients other partners.

Personal/Professional Development:
