Role Overview
As an Assistant Practitioner Nursing, the post holder is expected to independently manage their own work and case load, undertaking tasks delegated by a registered practitioner with appropriate supervision in place. Understanding evidence-based practice and delivering care in line with current evidence, the post holder will take responsibility for the provision of person-centred care in accordance with organisational policy and procedures.
Key Responsibilities
- Be responsible for some elements of patient assessment, undertaken in conjunction with the registrant, implementing programmes of care, and modifying programmes of care as delegated by a registered practitioner.
- The assistant practitioner will be allocated a group of patients to care for, similar to how patients are allocated to a registered nurse.
- The supervising registrant will undertake elements of care that sit outside of the band 4 scope of practice.
- Report regularly to the registered practitioner, recognising own limitations and seeking advice if they are unsure.
- Work in support of the multi-disciplinary team: registered nurses, doctors, and allied health professionals to provide a seamless service to patients with proximal supervision from a registered nurse/practitioner.

The team of assistant practitioners is vital to providing equitable access to care across Swansea Bay's most deprived areas where those at risk from viral hepatitis have limited access to health promotion, harm reduction, and specialist intervention. This team bridges the gap between community members and clinical nurse specialists leading elimination efforts by 2030.
Collaboration Efforts
The team will work effectively alongside BBV practitioners as well as Integrated Sexual Health, Respiratory, and Homeless Health teams to maximize each interaction's potential while delivering resource-efficient screening pathways leading towards specialist care.
Education Initiatives
Education and elimination projects undertaken by assistant practitioners involve locally innovated evidence-based high-intensity test-and-treat methods that deliver high-efficiency clinical interventions cost-effectively while addressing hidden infection burdens along with complex social stigma.
