Company Overview
The CRT Pharmacy team was set up in 2019 providing services to Newport, Caerphilly and Torfaen. Due to the continued success of the service, further investment in 2023 allowed an ambitious expansion to extend the service to all 5 boroughs and to redesign the pharmacy service to rehabilitation wards in County and Chepstow Community Hospitals.

Position Overview
One of our Clinical Pharmacist posts has become vacant giving an opportunity to join our rapidly developing Intermediate Care team. The ability to speak Welsh is desirable for this post; Welsh and/or English speakers are equally welcome to apply.

Service Description
The team currently provides a Pharmacy service to:
- Frailty Consultant-led hospital at home patients
- Therapies/Social Care-led Reablement patients
- Nurse-led home parenteral service patients (including IV antimicrobials)
- Community Falls patients

We take referrals from acute ward Pharmacy staff to ensure that frail/vulnerable patients are supported in taking medicines post discharge. The new funding allowed the exciting integration and ongoing redesign of the Pharmacy service provided to the community hospital rehabilitation wards of South Monmouthshire and Torfaen.

Role Responsibilities
Our role involves:
- Regular MDT meetings
- Home visits
- Virtual & actual consultant led ward rounds
- Data collection and analysis of contributions to patient care

Clinical Commitment
This clinical pharmacist post based in Newport CRT also has a commitment to the Chepstow Community Hospital rehabilitation ward which offers an opportunity for integrated health and social care teamwork. The role is supported by excellent Pharmacy Technicians working at the top of their licence pushing at boundaries of responsibilities.

With a unique role working across primary/secondary care interface we have built excellent relationships with community, practice-based, and acute hospital colleagues. The team is truly delivering 'A Healthier Wales'.
