Overview:
The Harrow Community Mental Health Triage and Assessment Team is looking for a passionate and experienced RMN who is eager to make a difference in an environment that promotes staff development and training opportunities. Service Focus:
Harrow is a pioneering service in Trauma Informed Approaches. We seek an experienced practitioner looking for a new venture in the community to further develop their skills, with training and development opportunities tailored to suit individual needs.

Role Responsibilities:
- Accountable for the RMN intervention of a defined caseload of adults.
- Assess health and social care needs (including accommodation, finances)
- Negotiate care plans with service users and their carers while remaining objective during diagnostic uncertainty.

Key Responsibilities:
Clinical Responsibilities:
1. Receive referrals from GPs; conduct comprehensive psycho-social mental health assessments.
2. Act as contact point/liaison role for GP practices regarding complex mental health issues.
3. Screen non-urgent GP Adult mental health referrals; identify suitable patients for primary care management based on agreed thresholds.
4. Facilitate onward referral/access rapid mental health services according to protocols/criteria; advise GPs on effective management strategies.
Communication & Liaison Responsibilities:
1. Serve as mental health resource offering advice/support for clients with difficulties within GP practices.
2. Maintain liaison between primary/secondary care clients known within mental health services; collaborate with key professionals/agencies involved in client welfare.
Training & Education Responsibilities:
1. Facilitate teaching/health promotion through education/advice/support provided directly within primary care settings (e.g., GPs).
2 Participate actively in teaching sessions aimed at enhancing knowledge among practice staff about detection/treatment of common conditions like post-natal depression.
Administrative & Professional Practice Duties:
1 Bring attention any situations requiring additional support/supervision immediately upon identification by informing Team Manager or Operational Manager accordingly .
2 Maintain accurate electronic patient notes using Systm One following CNWL/Social Services guidelines alongside Data Protection Act compliance .
Research & Development Participation :
1 Engage actively during learning sets adjusting processes reflecting developments observed .
2 Attend respective Primary Health Care meetings sharing insights gained improving information exchange between GP practices / secondary services .
Training Participation :
1 Engage fully clinical supervision alongside management supervision ensuring personal/professional growth aligned Personal Development Plan objectives .
2 Contribute towards induction/training initiatives acting mentor/supervisor students fostering conducive learning environments .
Policy Contribution :
1 Work collaboratively contributing towards developing policies/procedures aligning operational standards set forth CMHT framework .
2 Participate discussions expanding team dynamics liaising effectively community agencies enhancing overall service delivery outcomes .
