Opportunity Overview
We have an exciting opportunity for an experienced nurse to join the Neuro Rehabilitation Unit at Neath Port Talbot Hospital. The post holder will provide specialist nursing care to people with neurological conditions who are referred into the Neuro Rehabilitation Unit.
Key Responsibilities
- Provide comprehensive specialist, multifactorial assessment, plan, implement and evaluate evidenced-based nursing care.
- Engage in autonomous, independent clinical practice and work closely with Neurology consultants and clinical nursing staff within the department.
- Provide specialist education and training to staff.
- Carry a varied case load of neuro-rehabilitation patients with a variety of presentations, multi pathologies, severe physical and cognitive impairment.
- Work collaboratively and co-operatively with multidisciplinary clinical colleagues to ensure comprehensive care for patients.
- Undertake audit, support clinicians with their clinics, do ward-based work when needed and undertake telephone consultations as required.
Role Development Responsibilities
- Responsible for the development of the nursing role in the Neuro-rehabilitation service including junior nurses or AHPs and rehabilitation assistants.
- Responsible for screening referrals, presenting at multidisciplinary team meetings, arranging admissions and discharges, prioritising patients on the waiting list.
Clinical Responsibilities
- Assess differentially diagnose rehabilitate and manage a case load of adults with neurological conditions.
- Undertake highly specialist assessment of neurological conditions; carry out diagnostic procedures; interpret results for neurology patients with multi-faceted problems including those with highly complex presentations.
- Utilize clinical reasoning skills along with knowledge of evidence-based practice to determine appropriate care plans using specialist skills.
Patient Education & Support
- Provide comprehensive education and support to patients and healthcare providers.
- Participate in ward rounds outpatient clinics multidisciplinary meetings.
- Provide specialist clinical knowledge to colleagues patients carers/relatives.
Communication & Safety
- Establish effective communication channels with patients carers/relatives professionals across health social services.
- Seek appropriate advice/alert agencies regarding potential/actual risk concerning any patient or their dependents (e.g., Child Safeguarding domestic violence protection of vulnerable adults mental capacity deprivation of liberty).
