We are looking to recruit a Qualified Social Worker in the Hospital Discharge Team within Wandsworth Council. The successful candidate will be working in a well-structured, organised team who are growing their social worker staffing levels.

Responsibilities:
- Experience of working in a hospital setting.
- Experience of Mental Capacity Assessments/Hospital Discharge Assessments.
- Experience of Safeguarding.
Ideal Candidate:
- Social Work England registration.
- Eligibility to work in the UK.
- Social Work qualified (MA/BSc/DPSW/CQSW/MSc/BA).
