Description

Responsibilities:

- Form an integral part of the multidisciplinary team in caring for the physical, social and psychological needs of patients and carers diagnosed with Urological conditions throughout their treatment pathway.
- Have daily direct contact with patients and their families, providing teaching, support, advice and signposting.
- Undertake clinical tasks deemed suitable/appropriate to the role by the Clinical Nurse Specialists (CNSs).
- Work as part of the Urology multidisciplinary team under the guidance and supervision of a registered practitioner.

Key Duties:

- Provide patient-facing support for patients with Urological conditions.
- Be a direct point of contact for patients on the 28 faster diagnosis initiative.
- Triaging telephone calls and providing basic advice and support.
- Liaise and refer to other appropriate healthcare professionals especially within the community.
- Arrange and support clinics with Intravesical treatments.
- Complete flow and scan clinics alongside TWOCs (Trial Without Catheter).
- Arrange and triage electronic Holistic Needs Assessments (eHNA) during the cancer patient pathway. Assist patients in completing their eHNA where appropriate to ensure timely signposting to services.

Collaboration:

- Ensure appropriate management of patient information and coordination of patient pathways through collaborative working with medical secretaries, MDT coordinators, clinical nurse specialists, and other departments as required within the Trust.

Development Initiatives:

- Work with CNSs to develop and embed Living With And Beyond Cancer agenda within Urology service.
- Assist in developing end-of-treatment summaries alongside lead CNS for timely communication of patient information to primary care setting.

Administrative Responsibilities:

- Ensure appropriate booking of outpatient appointments.
- Maintain development of Urology CNS patient database including remote monitoring.
- Work closely with MDT coordinator ensuring roles are well defined without duplication.

Quality Assurance:

- Monitor administrative functions ensuring they are carried out to highest quality standards by improving services.
- Monitor patient's progress through cancer pathway identifying areas where targets are not achieved; undertake process mapping to improve performance.

Personal Development:

- Participate in learning relevant technical competency/area; keep up-to-date on local/national issues/developments.
- Manage own learning related to clinical area including mandatory training; participate in regular formal supervision developing personal development plan linking into KSF & departmental competency frameworks promoting learning/enhancing skills.

Time Management:

- Be responsible for managing own daily timetable including time for supervision/training.
