Role Overview:
As a Transformation Manager, you will play a key role in shaping and delivering clinically appropriate and sustainable services in Ealing, Hounslow and Hammersmith & Fulham. Supporting the senior transformation manager, you will be working on given transformation assignments to transform pathways, service models and deliver strategic priorities.
Responsibilities:
- Collaborate with senior clinical and operational leads to manage end-to-end transformation initiatives.
- Provide expertise, leadership, and robust processes to deliver improved outcomes for patients and carers.
- Support activities from co-production with stakeholders to ensuring estates, digital, and other enabling solutions are established for all initiatives.
- Manage projects that impact clinical services while maintaining effective communication with staff at all levels, partner organisations, and local system leads.

Key Workstreams:
This job holder will manage key transformation workstreams across Local and Specialist Services CSU aligned with national policy frameworks (including NHS Long Term Plan) as well as local drivers of change at Trust level.
Change Management Processes:
You will ensure robust change management processes are delivered by managing assigned projects including starting, maintaining, supporting, facilitating monitoring progress closing and evaluating multiple workstreams.
Project Management Input:
Provide expert project management input across the transformation portfolio including co-production with service users; enabling digital solutions; tracking outcomes; optimising workforce.
Collaboration:
Work closely with colleagues across Local and Specialist Services CSU including senior management within service area; clinical leads; support services; borough leads; NWL ICB Programme colleagues ensuring full involvement in all transformation work.
