Description

The Paediatric Diabetes Team is a small MDT team who provide high standards of care for approximately 100 CYP with diabetes. The team are based within the Childrens Centre of Dorset County Hospital, and have close links with the inpatient ward, Paediatric Assessment Unit and medical team, providing experience and leadership in all aspects of paediatric diabetes nursing care across the paediatric workforce.

Responsibilities:
- Provide specialist diabetes nursing care, working across inpatient, outpatient and community settings throughout the West Dorset locality.
- Act as an autonomous practitioner alongside the Paediatric Diabetes team, including clinical lead and medical colleagues.
- Responsible for assessment, primary and/or secondary diagnosis, management and evaluation of treatment and care for a caseload of children & young people aged 0-18 years with diabetes in both hospital and community settings.
- Provide nursing care for a defined caseload while educating and supporting children/young people and their families/carers in diabetes self-management skills from diagnosis to transition.
- Co-ordinate and implement an overall strategy of evidence-based care for the whole caseload of patients.
- Co-ordinate service alongside Clinical Lead and lead practitioners for medicine, nursing, dietetics, and psychology.
- Be a highly visible resource for specialist advice to professionals caring for children/young people with diabetes.

Leadership:
- Through effective leadership ensure that the clinical multidisciplinary team delivers a safe, effective responsive service where compassion is embedded leading to positive patient experiences.
- Provide strong professional clinical managerial leadership acting as a professional role model.

Collaboration:
- Work across organisational boundaries collaborating with health professionals in community services as well as external agencies such as social care education voluntary sector.

Expertise:
- Act as an expert resource in Paediatric Diabetes possessing extensive specialist knowledge to educate healthcare professionals CYP their families carers (including educational settings).

Education Responsibilities:
- Operate as Nursing Lead for Education overseen by Nursing Leads incorporating education requirements for ward staff schools patient education programmes.
- Collaborate with Clinical Lead service leads (e.g., medicine dietetics psychology) to deliver services meeting national standards such as RCPCH National Diabetes Quality programme currently PEER Review BPTA.

Prescribing Responsibilities:
- As a nurse prescriber (willing to undertake), demonstrate advanced knowledge experience using Oral Hypoglycaemic Agents (OHAs) insulin other treatment regimens relating to diabetes.
- Advise on stabilisation patients on all oral agents insulin other injected therapies according national regional locally formulated guidelines protocols.
- Demonstrate advanced knowledge experience prescribing treatments within scope practice alongside local policy.

Skills Required:
You should possess the ability to perform assessments of patient needs liaising with other members of multi-disciplinary teams where necessary.
